How to Request DSC Approval

In-personRequest at the court window, 1120 D Street, Floresville, TX 78114 — forms are provided at the window, so the mail packet forms are not needed for in-person requests.
MailMail the completed Request for a Driving Safety Course form (with plea of Guilty or No Contest circled), notarized affidavit, photocopies of Texas driver's license and insurance card, and payment to Floresville Municipal Court, 1120 D Street, Floresville, TX 78114. Do not mail cash.

To Request Approval — Submit These

  • Completed Request for a Driving Safety Course form with Guilty or No Contest plea, signed and dated
  • Notarized Affidavit for Driving Safety Course stating you have not taken/completed a DSC in the 12 months preceding the offense
  • Photocopy of valid Texas driver's license (military exempt from TX license requirement)
  • Photocopy of current insurance card listing you as a covered driver
  • Payment of court cost ($144 / $169 school zone)

After Completing the Course — Submit These

  • uniform certificate of completion AND certified copy of DPS driving record (Type 3A) — both due by the 90th day

Important Deadlines & Local Rules

  • Request deadline: On or before your initial/court appearance date shown on the citation
  • Completion window: 90 days from the request — certificate of completion, affidavit, and certified DPS driving record must be received by the court on or before the 90th day; no extensions granted.
  • DO NOT take the course before receiving permission from the court, and do not pay online until the court has granted permission.
  • The affidavit (no course in past 12 months) must be notarized for mail-in requests; in-person requests are signed at the window.
  • If paperwork is turned in early, the case is dismissed early.
  • Not eligible: CDL holders at time of offense, speeding 25+ mph over limit, passing a school bus, reckless driving, hit and run, construction zone violations, no insurance, or license/plate/inspection violations.
