How to Request DSC Approval
To Request Approval — Submit These
- •Completed Driver Safety Course Request form with plea of Guilty or No Contest
- •Notarized Driving Safety Course Affidavit (stating no DSC taken in previous 12 months and no CDL)
After Completing the Course — Submit These
- •certificate of completion from a TEA/TDLR-approved course
- •certified copy of your driving record from the Texas Department of Public Safety
Important Deadlines & Local Rules
- •Request deadline: Required items must be submitted prior to your initial appearance date, or you must speak with the Judge on your scheduled court date and time; late or incomplete requests are returned
- •Completion window: 90 days — form states 'ACCORDING TO STATE LAW: NO EXTENSIONS CAN BE GIVEN TO COMPLETE THE CLASS'.
- •Request form requires a notarized affidavit — plan for notarization before submitting.
- •Form quote: 'FAILURE TO FINISH THE COURSE BY THE REQUIRED DATE AND SUBMIT THE COMPLETION CERTIFICATE AND DRIVING RECORD WILL RESULT IN ASSESSMENT OF THE REMAINING FINE AMOUNT AND A CONVICTION ON MY DRIVING RECORD.'
