How to Request DSC Approval
To Request Approval — Submit These
- •Completed Driving Safety Course Request Form (must be notarized unless you appear in person; includes a no-contest plea and sworn affidavit)
- •Copy of a valid Texas driver's license or permit
- •Copy of proof of current vehicle insurance
After Completing the Course — Submit These
- •DSC Certificate of Completion and a certified copy of your Texas DPS driving record, submitted within 90 days of the approval date
Important Deadlines & Local Rules
- •Request deadline: On or before the appearance date noted on your citation
- •Completion window: 90 days from the court's approval date — do not start the course until the court approves the request and sends an instruction letter.
- •Verbatim form language: 'This form must be notarized unless you appear in person.'
- •Court explicitly warns: do not start the course until the court approves the request and you receive your instruction letter
- •Ineligible if: DSC taken within 12 months of citation, CDL held at time of citation or now, speeding 25+ mph over limit, speeding in a construction zone with workers present, passing a school bus, accident with vehicle damage, or failure to give information/render aid
- •Phone requests are not accepted — requests must be in person, by mail, fax, or online
