How to Request DSC Approval

MailDeer Park Municipal Court, PO Box 700, Deer Park, TX 77536 — mail copies of valid TX driver's license, insurance card showing defendant's name, the signed and notarized affidavit/application, and full payment of court costs

To Request Approval — Submit These

  • Completed and notarized Application for Driving Safety Course / affidavit (waiving jury trial, pleading no contest)
  • Copy of valid Texas driver's license or permit (exception for active duty military and military dependents)
  • Proof of current financial responsibility (insurance) with the defendant's name on the policy or card
  • Payment of court costs in full

After Completing the Course — Submit These

  • certificate of completion from a TDLR/TEA-approved course AND certified Type 3A driving record from DPS — both must be turned in at the same time; one will not be accepted without the other

Important Deadlines & Local Rules

  • Request deadline: Request must be made on or before the appearance date listed on the citation (before your court date)
  • Completion window: 90 days from the date the court approves the application — both the certificate of completion and the certified driving record are due within that 90 days.
  • Verbatim from court form: "DO NOT TAKE THE COURSE UNTIL THIS APPLICATION HAS BEEN APPROVED" — Deer Park requires court approval before enrolling in the course
  • Verbatim: "The driving record and certificate of completion must be turned in at the same time. One form will not be accepted without the other."
  • Not eligible if: course taken within 12 months of citation date, failure to stop and render aid, violation in a construction zone with workers present, speeding 25+ mph over limit, passing a school bus loading/unloading, or holder of a commercial driver's license
  • Juveniles age 16 and under must appear before the judge with a parent or legal guardian to request DSC
