How to Request DSC Approval
To Request Approval — Submit These
- •Completed Form DD-205 'Affidavit and Request for Driver or Motorcycle Safety Course' — notarized, or signature witnessed in person by the Court Clerk
- •Copy of valid Texas driver's license or permit
- •Proof of financial responsibility (auto insurance) listing you as a driver
- •Plea of Guilty or No Contest (entered on the affidavit)
After Completing the Course — Submit These
- •TDLR-approved course Certificate of Completion
- •Certified Type 3A driving record from Texas DPS (returned with the certificate)
Important Deadlines & Local Rules
- •Request deadline: On or before the court date (the appearance date on your citation)
- •Completion window: 90 days from the date the request is granted — certificate and Type 3A driving record must be returned to the court within 90 days.
- •ADDRESS CORRECTION: Court has moved to 2402 McKinney St., Melissa, TX 75454; 3411 Barker Ave. is now the mailing address. Current page is cityofmelissa.com/242/Municipal-Court.
- •Form DD-205 verbatim: 'You must receive the court's permission BEFORE taking the course. Please allow 48-72 hours to process your request.'
- •Signature may be witnessed by the court clerk if submitted in person; must be notarized if submitted online or by mail.
- •Not eligible if: speeding 25+ mph over limit, construction zone with workers present, passing a school bus, leaving the scene of an accident, CDL holder at time of offense, or completed a DSC within the 12 months before the citation date.
