To Request Approval — Submit These

  • Notarized Sworn Request for Driving Safety Course form
  • Copy of valid Texas driver's license or permit
  • Copy of valid proof of financial responsibility (liability insurance)
  • Signed affidavit stating no DSC taken for ticket dismissal in the 12 months prior to the citation date

After Completing the Course — Submit These

  • TEA/TDLR-approved DSC certificate of completion
  • Certified copy (Type 3A) of driving record from Texas DPS

Important Deadlines & Local Rules

  • Request deadline: Request must be made prior to the due date noted on your citation
  • Completion window: 90 days to complete the course and submit the completion certificate and certified driving record to the court.
  • Sworn Request form must be notarized.
  • Course must be approved by TEA/TDLR — find approved providers at tdlr.texas.gov.
  • Ineligible if: DSC taken within past 12 months, holder of a commercial driver's license, speeding 25+ mph over the limit, or charged with accident-related offenses, reckless driving, or passing a school bus.
